Do I need an appointment to speak to an Advisor?
It will depend on what your enquiry is about. If you require help to complete a benefit form then yes, you will probably need an appointment. Please ring or e-mail us for the next available date. If you need general benefit advice we may be able to help you over the telephone.
What should I do if my PIP form is due back soon but I need more time to complete it?
You should telephone PIP to request an extension on your return date. PIP should grant you an extra few weeks to get the form back to them. We advise you send the form back at least one week before the return date so it has time to arrive.
Can I pick a new form up from the Centre?
We have the following forms available in our office:
Attendance Allowance, Social Fund Loan, Sure Start Maternity Grant, Carers Allowance, Social Fund Funeral Payment. If you need any of these forms please call in to collect one between 10am and 4pm.
Personal Independence Payment forms can only be obtained from the DWP. You can telephone for a form on 0800 917 2222. The DWP will ask for your telephone number, date of birth, national insurance number, bank account number, your doctors details, dates and addresses for any time you have spent abroad, in a care home or hospital. They will then post a form out to you with a date when they need to receive it back by.
